Guidelines for “I am my First Client"
Due on December 10

Please follow these specific guidelines for submission of your “I am my First Client” profiles. Bold and Label Each Category
Paper should be typed and neatly presented.

Note: For “I am my First Client” you will only be presenting assessment information: no exercise prescription is required for this project.

I. Cover Page: Title (Such as I Am My First Client), Your Name, Designs for Fitness

II. Health/Lifestyle Assessments To Include (No need to summarize, just include) (1 pt)
A. Completed Medical History Questionnaire
B. Completed Lifestyle Evaluation
C. Cholesterol Summary
D. Risk Factor Assessment: Show number of possitive and/or negative risks factors.

III. Results of one Field Cardiorespiratory Fitness Assessment (.5 pt)
A. Show calculations (can do this by hand)
B. Classify your cardiorespiratory fitness level
C. You may do a submax or max protocol (as a substitute for the field test) but this will not receive any extra credit (show your calculations for determination of max and classify your fitness level)

IV. Choose one body composition method and assess the following (.5 pt)
A. Percent body fat
B. Fat mass (kg)
C. Fat-Free mass (kg)
D. Make sure you identify the body composition method (not need to show calculations here)
E. Determine your BMI
F. Classify yourself in %BF and BMI

V. Muscular Strength Assessment (.5 pt)
A. Choose 7 areas of the body (i.e., arm flexors, arm extensors, leg extensors, leg flexors, shoulder abductors, shoulder adductors)
B. Get a 1 repetition maximum or 5 repetition maximum for each or the areas selected
C. Identify the areas and present the results

VI. Flexibility Assessment (.5 pt)
A. Using the goniometer, find the following degrees of motion: shoulder flexion, shoulder extension, hip flexion, hip extension
B. Determine your sit-and-reach
C. Present and classify results

VII. Short Summary Essay (100 - 150 words) (2.0 pts)
Describe the purpose of fitness assessment and then explain why it is important for the exercise scientist to actually experience the same tests that future clients will be asked to do.
